The kernel is invoked with different parameters for "normal" and "safe" mode. The fail safe mode seems to give a more meaningful error message.
As the message states: it is unable to find the "init" program on /dev/hda5. Use Mandrake CD1 to boot in "rescue" mode and see if you can repair/mount hda5. Also strongly suggest make back up of your data files.

This error generally comes when you have installed your /boot or / partition on a filesystem for which drivers donot get loaded in the kernel. For eg : if you do a minimal installation of any distro and skip taking ext3 modules and ... you format your / or /boot with ext3 then you are bound to get this error!
If possible try booting from you cd and check for the fs types
