Monitor : Samtel sv410 At least this is what written at the back of the monitor, but Linux ( & windows also ) detect it as SAM d556)

Graphics Card : sis6326 with 4MB ram.

Will mail you my XF86Config file tommorow. My home machine uses Sam D556 and SIS6326. The card to be selected is no. 621 (IIRC). Works perfectly 1024x768 24 bpp and 1280x1024 24bpp with slight distortion on the l & r edges.
