Definately squid, there are others (many of them) try sourceforge.net or freshmeat.net

You need a windows 2k advanced server (may be NT also has it but I do not know), there is active directory service avaiable on win 2k which acts as a domin controller. As far as windows goes, this is where my knowledge ends.
