Most likely your HDD is on it's way out, even though it may be checking out fine later. I suggest you replace the HDD. I have seen SCSI disks appear and disappear on HBA scans - when they appeared they worked fine but ... they eventually RIP'd.
Also, I presume you have looked at cooling issues - overheating can cause weird symptoms.
